Even though a soap note may seem very simple to write out, it is actually very difficult to perfect. I am sure it is going to take time, and I will eventually get better that writing soap notes, but for something that seemed so direct and simple, when first introduced, it turned out to be a bit confusing, and I know it wasn’t just me because other people were confused as well. I think the most difficult part would be the objective verses assessment, just knowing what belongs where, is going to take some practice. Overall, I think if I am able to really perfect my soap notes, its an extremely helpful document for patient care
